Important Safety Information for Compounded Semaglutide (Injection)

Warning: Risk of Thyroid C-Cell Tumors

Semaglutide has been shown to cause thyroid C-cell tumors, including medullary thyroid carcinoma (MTC), in animal studies. It is not known whether semaglutide causes such tumors in humans.

Do not use semaglutide if you or a family member has a history of medullary thyroid carcinoma (MTC), or if you have a condition called Multiple Endocrine Neoplasia syndrome type 2 (MEN 2). Contact your provider immediately if you notice a lump or swelling in your neck, hoarseness, difficulty swallowing, or shortness of breath.

What Is Compounded Semaglutide?

This is a compounded formulation of semaglutide prescribed as part of a structured, clinician-guided health protocol for weight management and metabolic optimization. It is intended for adults with obesity (BMI ≥30) or overweight (BMI ≥27) with one or more weight-related medical conditions.

This product is prepared in a U.S.-based, FDA-registered 503A or 503B compounding pharmacy. It is not FDA-approved, but is dispensed legally with a prescription and clinical oversight.

Do Not Use Semaglutide If You:

  • Have a personal or family history of medullary thyroid carcinoma (MTC)

  • Have Multiple Endocrine Neoplasia syndrome type 2 (MEN 2)

  • Have had a serious allergic reaction to semaglutide or any ingredients in the compound

Tell Your Provider Before Use If You Have:

  • Type 1 or type 2 diabetes

  • A history of thyroid cancer, pancreatitis, or gastrointestinal disease

  • Kidney impairment or dehydration risk

  • Eye disease related to diabetes (retinopathy)

  • Depression or suicidal thoughts

  • Are pregnant, planning to become pregnant, or currently breastfeeding

Semaglutide and Pregnancy

Semaglutide should not be used during pregnancy. There is no clinical benefit to weight loss during pregnancy and it may cause harm to the unborn baby. If pregnancy is confirmed, discontinue use immediately and notify your provider.

Women of reproductive potential should use effective contraception. Oral hormonal contraceptives may be less effective while taking semaglutide. Non-oral or barrier methods are recommended during the dose escalation phase.

Drug Interactions and Precautions

Be sure to disclose all current medications, including:

  • Diabetes drugs such as insulin or sulfonylureas (may increase hypoglycemia risk)

  • Oral contraceptives (absorption may be affected due to slowed gastric emptying)

  • Other GLP-1 receptor agonists (e.g., Ozempic, Wegovy, Saxenda, Victoza, Byetta)

  • Weight loss medications or supplements (should not be used concurrently)

Semaglutide delays gastric emptying and may impact the timing of other oral medications. Your provider will help you adjust dosing schedules if needed.

How to Use

Administer compounded semaglutide as a subcutaneous injection once weekly on the same day each week, in the abdomen, thigh, or upper arm. Follow your provider’s dosage instructions exactly. Dosing typically begins low and is increased gradually based on tolerance and progress.

Serious Risks to Watch For

Call your provider or seek emergency care if you experience any of the following:

  • Thyroid tumors or neck swelling

  • Severe abdominal pain (especially if radiating to the back – pancreatitis risk)

  • Nausea, vomiting, or diarrhea leading to signs of dehydration (kidney risk)

  • Gallbladder disease symptoms: upper abdominal pain, jaundice, pale stools

  • Vision changes (possible complication of improved glucose control in diabetics)

  • Mood changes or suicidal thoughts

  • Severe allergic reactions (swelling, rash, difficulty breathing or swallowing)

  • Signs of low blood sugar (shakiness, sweating, confusion, fast heartbeat) – especially if taking other diabetes medications

Most Common Side Effects Include:

  • Nausea

  • Diarrhea

  • Vomiting

  • Constipation

  • Stomach pain

  • Fatigue

  • Acid reflux or belching

  • Headache

  • Mild injection site reactions

  • Mild hair thinning

Additional Safety Notes

  • Do not share injection pens or needles to avoid infections.

  • Discuss upcoming surgeries with your provider, as semaglutide delays gastric emptying and may increase risk of aspiration during anesthesia.

  • Monitor hydration, especially during the first weeks of treatment or dose increases.

Monthly Billing Structure
This treatment plan operates on a recurring monthly basis. By enrolling, you authorize AdeptRx to bill your selected payment method every 30 days. Your initial payment covers:

-Clinical intake and medical evaluation

-Access to the ADEPT-Rx digital platform

-If approved, the first months prescription of compounded tirzepatide and pharmacy dispensing

Platform access and continued participation require an active subscription.

Cancellation Policy
You may cancel future billing cycles at any time through your account portal before your next renewal. Cancellations apply to upcoming charges only. Once clinical review, shipment, or platform access has begun for the current cycle, that month remains active and is non-refundable.At the conclusion of your current billing period, cancellation will result in the removal of all platform access, including all app features.

Refunds & Clinical Ineligibility
Once your intake form has been submitted and provider review has begun, your payment becomes non-refundable, with the exception of ineligibility. If you are not approved for treatment, a partial refund will be issued minus a non-refundable clinical intake and administrative fee. This fee reflects the medical resources allocated regardless of outcome.
